Students Arriving at and Leaving School
Once students arrive at school, they may not leave without
permission from the office. After school, students must
leave campus and go directly home unless they are in a supervised
after-school activity.
When there is a change, even for the day, in the way a
child is to go home, that change must be given in writing
to the teacher or the school office. We cannot make a change
on a child’s say so.
When a parent needs to take a child from school during
the day, the parent or a person designated on the school
office contact card, must come into the office to sign the
student out. Only the parent and/or guardian may transport
a child from school. If a student has left school during
the day (with permission) and then returns, he/she must
check back in with the office to record the time of the
return. Students may not go home for lunch unless a parent
picks them up and signs them out in the office and then
in again upon their return.
Withdrawing from School
If you anticipate moving from our school area,
please come to the school office or call the office at least
one day prior to your leaving in order for the school secretary
to fill out a withdrawal form. This form needs to be signed
by a parent on the last day. All books must be returned
and all charges must be cleared at this time.

artificial coloring in the hair such as red, blue, green.

Attendance
Policy
Arizona State Law charges the parent or guardian with responsibility
for the student’s consistent school attendance. Attending
school each day, being on time, and remaining for the entire
day is very important for the total commitment of learning.
Absences ,tardiness and leaving early disrupts the learning
process. Parents are encouraged to make doctor, dentist,
and other appointments for students at times that will not
interfere with school. Students who miss two hours of instructional
classroom
time are considered absent for ½ day. Family trips
should be scheduled during non-school days. This will reinforce
the importance of school and will prevent the student from
getting behind in his/her work. Parents are discouraged
from taking students off campus early. Good attendance is
one of the most important habits your child can develop
in order to succeed in school and in life.
Madison Camelview School strictly enforces the laws regarding
attendance. Regular school attendance is essential for success
in school; therefore, absences shall be excused only for
unavoidable reasons. Such reasons include, bereavement,
observance of religious holidays, and illness. According
to Arizona State Law (15-803), absences exceeding 10% of
the school year are considered excessive. Children with
excessive excused absences may be asked to bring a doctor’s
note for each day absent in order for the absence to continue
to be considered an excused absence.
If a student is going to be absent from school, a parent
or guardian must call the attendance office at 664-7220
(NOT their child’s teacher, NOT the school office)
before 10:30 AM that day; otherwise, we will have to call
the parent at home or work to verify the reason for the
absence (State Law ARS 15-807). Absences not reported within
48 hours will be considered unexcused or truant. If a note
is submitted upon the student’s return, it too must
be received within 48 hours of the absence. For your convenience,
voicemail is available at 602-664-7220, 24 hours a day,
to inform the school of an absence. Please make sure you
indicate the reason for the absence if you use the voicemail
system. Student initiated calls will not be accepted.
Students whose parents do not excuse the absence will be
considered truant. A truant absence is one for which the
school has
received neither a written note nor a telephone call from
a parent or guardian within 48 hours of the absence. The
school will send a warning letter after the 3rd unexcused
absence. If the student has more than three unexcused absences,
a parent will be required to attend a meeting with school
personnel. Continued truancies may result in referral to
a truancy officer.
Students are expected to attend a minimum of 90% of the
school days per year. Each student’s attendance record
will be assessed individually as to the impact it has on
the student’s success in school, and is a fact when
considering promotion or retention. (IKE-R [2] and ARS 15-803C)
If a student is absent all day (excused OR unexcused), he/she
may not participate in extra curricular activities that
same day.
Tardy Policy
Habitual tardiness is a problem for the student (learning),
for the teacher and otherstudents (disruption of learning),
and the school (record keeping), and leads to other unsuccessful
experiences.
Students need at arrive to school in order to be seated
in their classroom by the time the second bell has rung
at 8:45 AM. All students arriving tardy (after 8:45) must
stop in the nurse’s office to receive a pass prior
to going to class.
Parents can excuse up to five tardies per semester. Tardies
of six or more per semester will be excused only with a
note from a medical office stating that the student had
an appointment, otherwise the tardy will be marked unexcused.
Each unexcused tardy may result in after school detentions.
In these cases parents will be responsible for transporting
students home from school. Students are eligible for
perfect attendance awards only if they have no tardies,
excused or unexcused.

Nurse Information
School Nurse: Diana Dellis (602) 664-7220
Do not send your child to school
if your child:
· Complains of a severe headache
· Has a fever of 100 or higher
· Vomits during the evening or night and still doesn’t
feel well in the morning
· Has yellow or green nasal discharge
· Has red or inflamed eyes
· Has any skin rash unless it has been diagnosed
by a physician as non-infectious
Any student with a temperature of 100.0 degrees or more
may not attend school. If a student is sent home from school
with a fever, he/she may not return to school until they
are fever free for twenty-four hours.
State law requires that students be excluded from school
if they are suspected of having a communicable disease.
Madison Health Offices are staffed by school nurses. In
the event of illness or accident during the school day,
the student will be referred to the school nurse. At the
time of registration and at the beginning of each school
year, the parent/guardian is requested to fill out an Emergency
Information card. The parent/guardian is required to furnish
the school with at least one telephone number where the
parent/guardian can be contactedduring the school day. PLEASE
NOTIFY THE SCHOOL OFFICE OF CHANGES IN YOUR TELEPHONE NUMBER
(EITHER WORK OR HOME) OR ADDRESS. IT IS IMPORTANT THAT WE
ARE ABLE TO CONTACT YOU AT ALL TIMES. If a child becomes
sick during the day, the school nurse will evaluate the
student and may need additional information or assistance.
Parents/guardians must make arrangements to have either
a parent, relative, or neighbor available at all times to
pick up a child who is ill.
We cannot keep an ill child in school. Please do not send
your child to school when ill. This request is made to:
· Protect the student because he/she is more susceptible
to secondary illness
· Protect other students from possible communicable
disease
Immunizations
The school maintains health records on each child. An immunization
record for school attendance must be completed and current
before a youngster can be enrolled. Arizona law (15-803)
requires that the parent prior to school attendance furnish
a complete, up-to-date immunization record. Every parent/guardian
must furnish the month, day, and year of the child’s
immunizations.
The immunization requirements for a child entering school
are:
Diphtheria Tetanus Whooping Cough Mumps
Measles Rubella (German measles) Hepatitis Polio
Parents are asked to inform the school nurse of any immunizations
received during the year so that the school records can
be kept up-to-date.
Medication
Prescription medication may be administered only at the
written request of the child’s physician. Medications
MUST BE in the original prescription bottle and kept by
the school nurse. There must be written permission from
the parent/guardian for the medication to be administered
at school. Over-the-counter medication may be administered
at the written request of the parent; it must be brought
in to the nurse in the original container. NO medications
in envelopes, foil, or baggies will be accepted. Medication
must be transported to and from school by an adult. Children
will not be allowed to transport medication.

Food

Nutritious hot and cold breakfasts and lunches are available
for students at school. Students may bring sack lunches,
however, we encourage parents to send healthy snacks such
as fruits, vegetables and juices. Candy, Hot Cheetos, and
Soda Pop is discouraged. Parents are welcome to join their
children at school for lunch any day. Menus are sent home
in advance each month.
All students will eat a free breakfast in the classroom
at the beginning of each school day. Free or reduced price
lunches are available to families who qualify for that assistance.
Applications are available in the school office.
Meal Prices
Meal K-3 Adult
Breakfast -1 day free $ 1.00
Regular Lunch – 1 day $1.75 $ 2.25
Weekly Lunch – 5 days $8.75 $11.25
Reduced Lunch $ .40 N/A
Weekly Reduced Lunch $2.00 N/A
These prices are subject to change.
To ensure compliance with Maricopa County Health Code our
school has established the following guidelines to offer
and encourage students to make safe, nutritious food choices:
v Foods for events such as student birthdays, class parties,
and other school sponsored functions should not be prepared
and cooked at home, as this is a violation of the Maricopa
County Health Code.
v Items brought to school from home for such functions should
be prepared by a retail establishment and pre-packaged for
each individual.
v Foods of good nutritional content including fruits, vegetables,
low-fat dairy foods, and lo-fat grain products, shall be
available wherever and whenever food is sold or otherwise
offered at school during the normal school day.
v Foods of Minimal Nutritional Value are discouraged. Foods
of minimal nutritional value as defined by USDA include
soda, chewing gum, hard candies, jellies, gums, marshmallow
candies, licorice, candy coated popcorn and hot cheetos.
v Foods and beverages that have as the first ingredient:
sugar, corn syrup, shortening, lard, or similar, may not
be sold or served to students on school grounds during the
normal school day.
Lunch Cards
All students will be issued a plastic debit card (looks
like a credit card) with their name, student ID number,
homeroom teacher, and their cafeteria account number on
the first day of school. Students depositing money to their
account must go the cafeteria cashier before school. Please
label payment with student’s first and last name.
Students using the lunch cards to purchase lunch instead
of cash will help speed the lines and provide a secure way
to handle money. Cash is easily lost, but the card system
will accurately deposit money to the student’s account,
give a balance each day, and provide a method for students
and parents to assure the student of a lunch. K-3 students
are allowed one ‘charge’ at a time if the balance
reaches $0 – as a courtesy to parents. If the lunch
card balance is $0 or negative, your child’s hand
will be stamped, notifying you to send money. Lost or damaged
lunch cards may be replaced for $1.00 through the cafeteria
cashier.
